Understanding Pelvic Congestion Syndrome

Pelvic congestion syndrome is a condition characterised by chronic pelvic pain, resulting from the pooling of blood in the veins of the pelvis or lower abdomen due to weakened venous valves.

Recognising Pelvic Congestion Syndrome

Pelvic congestion syndrome (PCS) is characterised by a dull ache or a sensation of heaviness in the pelvis that can persist for 3 to 6 months. The pain may occur on one or both sides of the pelvis, more commonly on the left, but it can alternate between sides. Factors that increase abdominal pressure, such as walking, posture changes, lifting, or prolonged standing, often intensify the pain. It tends to worsen before or during menstruation, with subsequent pregnancies, and during or after sexual activity. Pain intensity is typically greater at the end of the day.

Who is Affected?

PCS predominantly affects women aged 20–50 years, particularly after multiple pregnancies.

Types of PCS

  • Primary PCS: No identifiable cause.
  • Secondary PCS: Results from vein compression by nearby blood vessels, leading to congestion.

Diagnosis

  • Clinical evaluation
  • CT scan of the abdomen and pelvis

Treatment: Ovarian Vein Embolisation

Ovarian vein embolisation is a minimally invasive, nonsurgical day procedure performed under light intravenous sedation.

  • Procedure:
    • A small 2 mm catheter is inserted through the groin and guided into the pelvic veins under imaging.
    • The procedure takes about one hour.
  • Recovery:
    • Most patients report no pain post-procedure, with some experiencing mild discomfort for a day or two.
    • Normal activities can typically be resumed within a couple of days.

 

This effective and safe treatment offers relief from PCS symptoms, helping patients return to their daily lives with improved comfort.
